個人資料收集聲明

1. 你向香港電台所提供的資料，將用作書信往來、參與聯機形式調查、參與節目活動或處理你申請的有關事宜。

2. 你提供資料完全出於自願。如你不能提供所要求的資料又或者資料不正確或不全，香港電台可能無法以聯機形式處理你的個案。

3. 在某些情況下，你所提供的資料或會向政府其他決策局和部門，以及其他公司公開。

4. 查閱及更正個人資料的要求，須以書面提出。來信應寄交九龍廣播道 30號廣播大廈 3樓電台主任秘書收。

Personal Information Collection Statement

1. The personal information provided by you to Radio Television Hong Kong will be for purposes such as corresponding with us, participating in an on-line survey, entering a programme activity or processing your application.

2. The provision of personal information is voluntary. If you fail to provide the data requested or if the data provided is inaccurate or incomplete, we may not be able to process your case on-line.

3. The information provided by you may be disclosed to other Government bureaux and departments and other companies under certain circumstances.

4. Requests for personal data access and correction should be made by letter to the Departmental Secretary of Radio Television Hong Kong on 3/F, Broadcasting House, 30 Broadcast Drive, Kowloon.
